MEDICAL SOCIAL WORKER
Our Medical Social Workers help patients overcome some of the things that can get in the way of recovering from an illness or injury. Our Medical Social Workers perform home assessments to identify social, emotional, and financial concerns of the family & patient.
Our Medical Social services are inclusive of, but not limited to the following:
- Assistance navigating community resources
- Family Counseling
- Health insurance coverage education
- Alternative living arrangements
- Assist with financial concerns
- Long-term care planning
- Provide information and assist with Advance Directives
- Case management
- Bereavement support
